Winston-Salem, NC -- The N.C. Center for Missing Persons has issued a Silver Alert for a missing, endangered man Dwain Anthony Fischer.
Fischer, who is believed to be suffering from dementia or some other cognitive impairment, has been missing for two weeks.
The 43-year-old black man has long black hair, black eyes and is six foot, one inch tall and 160 pounds. He was last seen wearing a white t-shirt and blue jeans.
Winston-Salem Police say he was last seen at 129 Westdale Avenue in Winston-Salem.
Anyone with information about Dwain Anthony Fischer is asked to call J. P. Reyes at the Winston Salem Police Department at 336-773-7821.
